Case Management

Ageing and Disability Services (ADS) offers case management services to help seniors and persons with physical, cognitive or developmental disabilities who require assistance in their daily lives.  

Case management is a collaborative process of assessment, planning, assistance, care coordination, evaluation, and advocacy. ADS’ case management services help individuals and families identify options and secure available services necessary to meet their needs. Case management services include:

  1. Comprehensive needs assessment;
  2. Identifying required and available resources and supporting clients, or their relevant persons, to access them;
  3. Facilitating care coordination of existing support services.

How to get case management services

Anyone can request case management services directly (as the client or a family/friend of a senior or persons with disability) or through another service provider.

If you are a service provider (e.g. healthcare professional, seniors’ agency) and want to refer a client for case management please submit the
