The Department of Health offers comprehensive support through its Community Health Referral Services. Authorized medical professionals in Bermuda can submit a referral using the online form for these services: Nutrition Services, Long-Term Care Assessment, Social Worker, Community Health Nursing, or Allied Health Services.
