Asthma Registry of Bermuda Student
Help keep your child safe and supported at school. The Bermuda Student Asthma Registry lets parents and guardians share their child's asthma information directly with their school, so staff know how to respond if a flare-up happens.
This registry is a joint effort between the Department of Education, the Department of Health, the Bermuda Hospitals Board Asthma Education Centre, and Open Airways.

Why this matters
Asthma is one of the most common chronic conditions affecting school-aged children. When schools know which students have asthma — and how to support them — they can respond faster and more confidently in an emergency, and help students manage their condition day to day.
By registering your child, you're giving their school the information needed to:
- Recognize the signs of an asthma episode
- Follow the right steps to help your child
- Keep a record of relevant medical and emergency contact details
Before you start
You'll need the following information on hand:
- Your child's full name, date of birth, and school
- Your contact details as parent/guardian
- Your child's doctor's name
- Basic details about your child's asthma (as prompted in the form)
Who should complete this form: Parents or guardians of any child aged 4–18 with asthma.
A separate form is required for each child with asthma.
