This application is to change any client personal information for FutureCare and HIP policies; including name, date of birth and address. Released February 2011

- Premium Refund Form
This form is used for clients to received reimbursement for overpaid premium payments for outstanding balances for terminated accounts. Released March 2010
- Letter to Groups on Lapse & Termination Process
This letter advises all Employers and Self-Employed persons that the Health Insurance Department is implementing its lapse and termination process for arrears premiums. The termination process is a legal requirement under the Health Insurance Act 1970. Released December 2011.
