The form also includes authorization for the release of information from doctors, dentists, hospitals, the Bermuda Police Service, and employers. ​ The form must be completed in full and submitted within one year of the date of the injury. ​

To apply for compensation on behalf of dependents of a deceased person, you must complete the Application on Behalf of Dependents of a Deceased Person with Respect to the Compensation Payable to such Dependents form. ​ 

  1. The application must be completed in full and submitted within one year of the date of injury or death. ​ 
  2. In order to apply, you will need to provide personal information about the deceased, including their name, address, age, and occupation at the time of the incident. ​ 
  3. You will also need to provide details about the circumstances that caused the injury, the nature of the injury, and any hospital or dental treatment received. ​ 
  4. Additionally, you will need to provide information about the deceased's earnings, any benefits or compensation received as a result of the injury, and any previous applications for compensation. ​ 
  5. The application must be signed and dated, and you must agree to provide any necessary documentation and authorize relevant parties to release information to the Criminal Injuries Compensation Board. ​
